Insurance Information

A number of different insurance options are available to patients at NFC. We are able to bill most commercial insurance companies* for services provided at our clinic. In order to determine if your insurance plan provides benefits for naturopathic services please provide us with the following information:

1. Your full name
2. Date of Birth
3. Your phone number and email address
4. The name of your insurance company
5. The type of insurance plan you have (HMO, POS , PPO, etc.)
6. Your insurance subscriber number or member number (on the front of the card)
7. The member services telephone number for your insurance company (on the back of the card)

This information can be phoned in at 602-493-2273 or sent to our office via email. You may also photocopy the front and back of your insurance card, write your date of birth on the page and fax it to 602-493-2159 .

*Please note that Medicare does not cover services provided by naturopathic physicians, therefore, we are not able to bill Medicare claims.
